Upload a Photo ID
Your provider may ask you to share a Photo ID for billing and/or insurance purposes, as part of your intake process. This is different from uploading a profile picture for your account.

Uploading a photo ID
In order to upload a photo ID to your account, your provider must have sent you the Photo ID intake form. You can access this form:
- Through your email, where your provider has likely sent you a form completion request.
- From your Healthie dashboard, by clicking on the "Fill out Forms" button.
- By going to the "Forms" tab in the left hand side menu.
Once you have accessed the form, you can upload a photo of the front side of your photo ID.
If you need to change or update your Photo ID at any point, please reach out to your provider so they can send you a new form.
Needing to reach your provider?
If you would like to reach your provider, we recommend that you Message your Provider via Chat. Within your account, navigate to Chat > New Chat Message.
